Lifecare Family Health & Dental Center is dedicated to providing the best possible care and services to our patients. We are open to persons of all ages, regardless of ability to pay and health insurance status. Certified Application Counselors are available for assistance with completing eligibility applications for Medicaid and for Health Insurance Marketplace plans. MEDICAID

Lifecare is a participating provider with Medicaid and we accept all programs, including Buckeye Health Plan, CareSource, Molina Healthcare, United Healthcare and Paramount Health Care. We will submit claims for payment of services.

UNINSURED

Lifecare offers reduced rates for qualifying patients based on household size and income. If you need more information or are interested in applying for our Reduced Rate Program, download and print the Reduced Rate Application form.

MEDICARE

Lifecare is a participating provider with Medicare. We will bill Medicare for all of your covered charges and bill any secondary insurance plan you may have.

PRIVATE INSURANCE

Lifecare accepts most private insurance plans. Call Lifecare at 330-454-2000 for any questions you may have about your healthcare plan.

PAYMENTS ACCEPTED

Co-pays are due at the time of service unless other payment arrangements have been made in advance with our billing department at 234-521-7212. For your convenience, Lifecare accepts payment with a credit card, debit card, cash or personal check.
